What is a Person Initiated Follow Up?

A Person Initiated Follow-Up, or PIFU for short, puts you in control of your care.

You will be given direct access to our service for one year from the date of your last appointment.   

In the past, you may have been booked in for regular follow-up appointments with us, but a Person Initiated Follow Up means you can have more choice over how and when you access care if required

PIFU helps to personalise care based on what matters to you, your lifestyle, and circumstances when returning to our service.

What are the benefits of a Person Initiated Follow Up (PIFU) ?

  • PIFU enhances the opportunity for you to be directed to the right service at the right time. 
  • It gives you more control and choice over when you access your care. This means fewer unnecessary trips to clinic. 
  • It helps you and your clinician risk manage and make shared decisions together. 
  • It helps to give you the confidence, tools, and support to be able to manage your own care at home.

How do I access my Person Initiated Follow Up (PIFU) ?

If you have concerns or worries about the problem you spoke to us about, please contact us. A clinician will review the details and a member of the team will be in touch. We can contact you by telephone, or arrange to see you in one of our clinics.

A short questionnaire will be sent via text or emailed to you. You need to complete this so we can understand what you need. 

Alternatively, you can call our team who can assist you in completing your PIFU questionnaire.
